Each Facility has its own GP/nurse practioner team who will visit regularly and frequently.

We visit most facilities at least three times weekly. Outside these times we can visit or give advice as required.

We work in small supportive teams and hold regular meetings in the facilities in which we work. We invite facility staff to join us in our learning sessions.

A typical facility that uses our services:
Actually there is no typical facility. This is, however, a familiar story.
We are approached by a facility manager of a 70 bed unit who has been struggling to get quality GP visits for the last 6 months. Their previous GP has retired and the medical centre has instead sent a series of GPs all of whom are well-meaning but appear rushed, visit unannounced, often at residents mealtimes and are difficult to contact when there is a problem. Faxes are not replied to, and calls have to go through a receptionist. Frequently the medical centre is too busy to return calls for 24-48 hours. After hours and at weekends, there is difficulty accessing care even for palliative care. The facility RNs are at their wits end.
We meet with the facility and agree to visit 4 days each week for about 2 hours. Two of those visits will be by a GP and two by a NP. The NP and GP work closely together and are part of a study group which meets monthly at the facility. Together we institute the setting up of an electronic prescribing module. We target the improvement in the quality of 3 monthly resident assessments, along with an improvement in the relationship between the facility staff and medical care. We meet after 6 months to look at how the process is progressing and how we can progress. Out of hours the facility are able to ring straight through to a cell phone which is answered by either a GP or NP who has access to the patient notes.
